Adds a `triage` skill documenting how to investigate a GitHub issue or PR
and record a durable verdict, ahead of a sweep over the open PR backlog.

Write-ups live in the gitignored `.triage/` tree, one directory per ticket
(`.triage/issues/NNNN/`, `.triage/prs/NNNN/`), each holding `results.md`
plus whatever repros and fixtures the investigation needs. Every write-up
opens with YAML frontmatter so a 250-PR sweep stays greppable and can be
resumed from disk; `reindex.mjs` derives the index table from it.

The skill is discoverable by both Claude (`.claude/skills/`) and Codex
(via the `.codex/skills` symlink), with a pointer in AGENTS.md for agents
that don't auto-load skills.

Also included:

- `scripts/triage-signal.ts`, which ranks open issues and PRs by a
  synthesized engagement signal to prioritize the backlog. Committed here
  only because the pre-commit hook rejects any untracked file.
- Ignore `bun.lock` alongside the existing `bun.lockb`, and let `.fray/`
  self-ignore the way `.frizz/` already does.
