* server : address rerank multimodal review findings

- reject multimodal rerank input when the model has no rerank chat template (images were silently dropped)
- translate handle_media download/base64 failures to 400 instead of 500
- reject negative top_n with 400 instead of letting resize throw
- fix comment typos, redundant comments, indentation and trailing whitespace
- README: document that TEI input accepts the same shape as Jina; drop stray blank line
