* docs: add the v1.19.1 release note for the placement connection leak

Add docs/release_notes/v1.19.1.md with the entry for #10912. That
change closes the gRPC connection to Placement when a stream fails to
open or ends. It merged without a release note.

* docs: add the v1.18.10 release note for the placement connection leak

Add docs/release_notes/v1.18.10.md with the same entry as v1.19.1.md.
release-1.18 has the same tryConnect code, so the leak is there too.
The 1.18 note leaves out the goroutine growth rate, because that rate
was measured on master.
