jack 11552c42f3 fix(agent): cap parallel generate_tts calls and keep failure reasons (#1983) ...

Limit concurrent TTS requests per connection (managed id, or provider + base URL + key hash; configurable, default 1), carry Retry-After cooldowns across narration requests without applying them to quota refusals, and report classified failure codes in generate_tts / generate_actions results. Bumps @openmaic/generation to 0.3.27.
